How Can I Get Services?
Anyone can inquire about hospice services.  An ill person or their loved one may call a local hospice and request services.  The hospice will then contact your physician for approval.  Hospice may also be initiated by a physician, an assisted living facility, or a skilled nursing facility.
Make a Referral
The best time to learn about hospice is before it is needed.  You can discuss hospice care with a Hands of Hope Hospice staff member at any time by calling 208-523-7441.  You can also request an evaluation of a loved one at any time, free of charge, to see if he/she would qualify for services.

For a Referral:

A referral to hospice is generally appropriate when:

  •          A patient has a life-limiting illness, with a possible life expectancy of 6 months
  •          Comfort care and symptom management are the desired primary focus
  •          Curative treatment is no longer the patient’s choice or option.

Hospice services are available to patients without regard to disease, color, creed, national origin, age, sex or ability to pay.

Contact us by calling (208) 523-7441, or send an e-mail to info@handsofhopehospice.com.  A hospice nurse will then contact you to arrange an evaluation.  You can also fill out a REFERRAL form by clicking here.
